I understand that eliving offers products and services are intended to benefit people who have or may be at risk of certain health conditions, but that eliving products and services are available to everyone, regardless of health condition.
I understand and agree that my use of this website and my purchase of products and services from eliving does not require me to provide any specific health information, including, but not limited to, health history, family history, and specific diagnoses.
I agree that if I provide or disclose any health information to eliving, I will do so voluntarily. I understand that eliving will maintain the privacy of any health information that I do provide consistent with the eliving Privacy Policy.
I also understand and acknowledge that the information provided to me by eliving, whether on the Website or through any other form of communication, is in no way intended as medical advice or as a substitute for medical treatment. eliving’s products are not intended to diagnose, treat, cure or prevent any disease.I understand that any information that I receive from eliving should only be used in conjunction with the guidance and care of my physician; that the information provided by eliving is not intended to be a substitute for professional medical advice, diagnosis or treatment; that I should consult with my physician prior to beginning any nutrition or fitness plan and have my physician conduct proper follow-up visits to individualize my nutrition or fitness plan as appropriate; and that I should consult with my physician or other qualified health care provider if I have any questions regarding a medical condition, my diet, nutritional supplements, an exercise regimen or any other matter related to my health and well-being.
